I got a copy from Machinery Manuals. They charge an arm and a leg; like $150. Copy quality was pretty good. Tony Griffiths (spelling?) over in England at the Lathe Archive might be able to fix you up with a copy for less money but he could never seem to get around to making one for me. Are you restoring a machine? I have one that is waiting in storage until I can get to it.
