1, 2)Thermal relief is a negative pad (clear, surrounded by black), often created with a special aperture flash to connect a pin to an embedded metal layer that distributes a voltage, such as a power or ground.
If pin pass through plane layer, but not connect to this layer, for pad used "anti pad", which bigger than regular pad to isolate pin from plane. I send you e-mail with some examples in attachment...

3) Dimensions of TR & AP depends on electrical parameters of a net, design density and manufacturer's rules.
