The small transformer is a noise filter and has no bearing on the operating voltage. If the design is conservative (not likley) it will operate OK on 18 volts and produce 18/12*110=165VAC out. If you change any capacitors that are rated less than 25 volts, change the power transistors to some with double the voltage rating and rewind the transformer, It should be able to produce 18 VAC out with 18 VDC input.
