Subject: Replacement for bulky audio transformer?

I am searching a space saving replacement for linear audio transformers. It is used for electrical isolation in a phone application with differing ground potential on both sides of the "transformer".
Does anyone know a _single_ chip solution? Perhaps something like:
1. A/D converting
2. electrical isolated transfer of the digital
data (e.g. using a optocoupler)
3. D/A converting
