Murphy's Law states that if it is possible, it will happen, and at the most inoportune time. In a system having many ICs, each having many wire bonds which can break, the reliablity is low. There are so many failure modes, there is no point in enumerating them. The thing to do is design for low stress and burn-in parts to sort out early failures.
