I'm a bit puzzled because 46 dB represents a ratio, not a power level. However, if you want to drive an intercom speaker, a pretty good power level for that purpose would be one watt (+30 dBm) although I can't quantify how many dB SPL you would end up with from that.

If you don't have a circuit which would be suitable for this, National Semiconductor has a nice product line of these kinds of amplifiers. Take a look at: http://www.national.com/parametric/0,1850,799,00.html
